Job Description:
Agape Care Group is seeking a compassionate and experienced
Spiritual Care Counselor to join our dedicated team of healthcare
professionals. In this pivotal role, you will provide spiritual
support and counseling to patients and their families, facilitating
comfort and dignity in their final stages of life. As a Spiritual
Care Counselor, you will work closely with our interdisciplinary
team to ensure a holistic approach to end-of-life care that aligns
with the patient's spiritual, emotional, and psychosocial needs.
Conduct spiritual assessments of patients to determine their needs
and provide personalized care plans. Offer spiritual support and
counseling to patients and their families, addressing their
concerns and hopes. Collaborate with other healthcare professionals
to ensure an integrated approach to patient care. Develop and lead
spiritual programs and services such as prayer sessions, bible
studies, or meditation based on patient preferences. Document
patient interactions, progress, and outcomes in keeping with
department protocols and standards. Educate staff and families on
various spiritual care practices and their benefits in palliative
care settings. Participate in team meetings and contribute to the
care planning and review process. Excellent interpersonal and
communication skills. Strong empathy and listening skills. Ability
to provide spiritual guidance in a non-denominational manner.
Experience in crisis intervention and providing emotional support.

401k Board Certified Chaplain (BCC) by the Association of
Professional Chaplains or eligible for certification. Additional
training or certification in grief counseling or palliative care is
advantageous.
