Job Title: RN Case Manager Profession: Nursing Specialty: Case Management Duration: 11 weeks Shift: Monday - Friday Hours per Shift: 8 hours (8:00 AM - 4:30 PM) Experience: Minimum 2 years in acute inpatient management and discharge planning License: Required Certifications: BLS (must be AHA), ACN preferred Must-Have: - Acute inpatient management discharge planning experience - Epic experience - Preferred ICU and ER experience Description: The position involves managing a case load of approximately 20 per day. Experience with Epic EMR is required. The role includes discharge planning responsibilities. The case management team works collaboratively with insurance companies for approvals. Business casual attire is expected. The schedule may include working weekend shifts, with a flex day during the week. There is no on-call requirement. Holiday rotations are expected, including one major and one minor holiday. First to float to other units when necessary.
